What to Expect at Your First Psychological Therapy Session

Starting therapy can feel intimidating, especially if it’s your first time. However, knowing what to expect during your initial session can help ease any nerves and prepare you for a meaningful experience.

Your first session is primarily about getting to know each other. The therapist will typically begin by introducing themselves, their approach to therapy, and the confidentiality rules, including their limits (such as mandatory reporting for safety concerns). This is also your chance to ask any questions you have about how therapy works.

You’ll be asked about what brought you to therapy—whether it’s anxiety, depression, relationship issues, or something else. Don’t worry about having the “perfect” way to explain it. Your therapist will help guide the conversation and ask thoughtful questions to better understand your concerns, history, and goals. You might discuss your medical background, any previous experiences with therapy, and your current emotional state. There may be forms or questionnaires to fill out, especially if you’re in a clinic setting. These help the therapist get a broader picture of your mental health and well-being.

It’s normal to feel emotional, unsure, or even awkward. Therapy is a safe space, and there’s no pressure to share everything at once. Building trust takes time. You should feel empowered to ask questions and you can decline to answer any questions. You can share what you feel comfortable with and take things at your own pace.

The first session usually ends with a discussion about what future sessions might look like, how often you’ll meet, and what kind of goals you’ll work toward together. You may leave with some insights or simply with a sense of relief for having taken the first step.

Therapy is a journey that can be as long or as short as you want it to be. The first session is just the beginning. Being open, curious, and patient with the process can lead to profound personal growth over time.

Here are 4 practical and emotional tips to help you prepare for your first psychological therapy session:

1. Reflect on Why You’re Going Before your session, take a few minutes to think about what’s been bothering you. You don’t need to have all the answers or a perfect explanation. Just try to identify a few issues or feelings you’d like to explore—this could be stress, relationship problems, alcohol use, poor sleep, anxiety, low self-esteem, or even just a general sense of unhappiness.

2. Manage Your Expectation Therapy isn’t about instant fixes. The first session is usually about getting to know each other, not diving deep into solutions. It’s okay if you don’t feel dramatically better right away—real progress takes time and consistency. You may need more than one session to feel comfortable with your therapist. Building a therapeutic relationship can take a few sessions. However, you should always feel empowered to change to another therapist if you continue to feel uncomfortable or are unsure if the treatment plan is right for you.

3. Be Honest, Even If It Feels Uncomfortable You don’t have to share your entire life story in the first session, but being honest about how you’re feeling helps your therapist understand how to support you. If you’re nervous or unsure what to say, tell them—that’s part of the process too.

4. Give Yourself Time Afterwards Try not to schedule anything emotionally demanding right after your session. You may feel tired, emotional, or reflective. Giving yourself some quiet time afterward can help you process the experience and ease into the work ahead.
